I don't think they'd be allowed to have a child actress who looked 'starving', it would be unhealthy for them and really skinny actresses are bad role models. Because of this, whoevers casting the movie has to make sure that the actors are healthy so people who were thin enough to seem starving wouldn't get cast. I'm not having a go at your answer btw, sorry if it looks like that, I think its a great answer.
