Prim was burned to death by a flamable bomb that Gale and Beetee created in District 13. President Coin sent Prim out as the leader of a squadron of soldiers fighting againts the Capitol, and since Coin didn't like Katniss, she sent in planes with the bombs in them to kill Prim to put Katniss in her place.
